Forest Hills Holistic Health provides integrative and functional wellness services focused on education, lifestyle guidance, and root-cause exploration of health concerns. Services may include consultations, health reviews, functional lab interpretation, and personalized wellness recommendations.

These services are intended to support overall wellness and do not replace care from your primary care physician or licensed medical provider.

2. No Guarantees

While every effort is made to provide thoughtful, individualized care, results vary from person to person. No specific outcomes, improvements, or results are guaranteed.

Healing is a dynamic process influenced by many factors, including health history, lifestyle, environment, and consistency of care.

3. Client Responsibility

You understand that you are responsible for your own health decisions, including how you choose to implement any recommendations provided. You may accept or decline any suggestions at any time.

4. Payment & Cancellation

All fees for services are clearly communicated at the time of booking.

Appointments require advance notice for cancellation or rescheduling. Missed or late-cancelled appointments may be subject to a fee.

5. Communication

Communication outside of scheduled appointments may be limited. Non-urgent messages will be responded to within a reasonable timeframe.

For emergencies, please contact emergency medical services.

6. Intellectual Property

All materials provided during consultations or on this website are for personal use only and may not be copied, distributed, or shared without permission.
